Please complete the required forms below by their due dates. Please submit each form following the directions found on the bottom of each form. It is important that you consider your preparations well in advance and complete all required forms before your arrival.
High School Accepted Students

Due within 2 weeks of acceptance, along with $300 deposit. Please submit your deposit by mail to SEA High School Programs, PO Box 6, Woods Hole, MA 02543 or submit payment online. Please note: A 3% convenience fee will be applied to deposit payments made online.
Due no more than 6 months and no less than 1 month prior to your program start date.
You may also return forms by mail or fax, depending on their content.
